In order to ensure this philosophy for the future, the primary responsibility is to enhance and expedite the admissions process in our Grace owned and affiliated programs through strong community partnerships and presence in local hospitals that are looking to discharge patients.Benefits offered dependent on your work status: Competitive Pay Medical, Dental and Vision Health Savings Account Paid Time Off PayActiv - On demand access to earned wages Retirement match (up to 4%)ESSENTIAL FUNCTIONS: Obtain prescriptions from doctors as needed Clarify orders Assess residents per request Hire and train staff related to treatments needed by residents and medication pass Monitor monthly vital sheets and make recommendations to tenants regarding overall health Communicate to Director what follow-up will be needed over the week Check clipboard for resident requests Monitor diabetic blood sugars and make recommendations to tenants/physicians Diabetic cares as needed, i.e. fingernail and toenail care Checking lab test report sheets from Clinic, follow through with medication changes as needed On-call responsibilities Monitor/assess residents out of facility, i.e., hospital/nursing home Develop Individual Service Plans/Care Plans Audit charts Perform annual evaluations of nursing staff Monitor nursing staff performance Other duties as assigned.by Jobble
