Job Description:
A company is looking for an RN-Call Center in the Continuum of
Care.
Key Responsibilities
Staffs a 24-hour telephone service for hospice, home health, and
telehealth services
Collaborates with staff, patients, families, and other healthcare
providers to ensure effective patient care
Triage, evaluate, plan, and implement patient care plans within
nursing care standards
Required Qualifications, Training, and Education
Graduate of an accredited school of nursing or equivalent education
for licensing
Two years of progressive home care or hospice nursing experience or
equivalent education
Registered Nurse (RN) licensure in multiple states including WI,
IL, MO, and OK
Must obtain Illinois RN license within 3 months of hire
Experience in hospice and/or home health preferred
